- be alert to special situations

- the patient may hand you an insurance form requesting that only certain systems be examined

- the patient will tell you that you don’t need a history

- go ahead and introduce yourself, examine the systems listed, and then leave the room

Step 2 CS – How the examination is scored

- 10 out of 11 or 12 patient encounters will be scored

- an SP and a physician score each encounter

- SP fills out 3 checklists: one for history, one for physical exam, one for communication skills

- The physician will evaluate the PN you write

Overall score

- Integrated Clinical Encounter (ICE)

- ICE score = DG + PN

- ICE = Data-Gatherine + Patient Note

- ICE is composed of data-gathering score (DG) and patient note (PN) score

- final DG score will represent average of your performance with all 10 SPs

- If you asked questions or performed procedures that are not on an SP’s checklist, you will not receive credit but at the same time you will not lose credit for having done so.

- The PN score is grade based on organization, quality of information, data interpretation, legibility, and the absence of inappropriate or dangerous actions.

= Your final PN score will represent the average of all 10 encounters

Communication/Interpersonal Skills (CIS) score

- CIS score = IPS + SEP

- CIS score = Interpersonal Skills + Spoken English Proficiency

- IPS includes rapport, interviewing skills, personal manner, and counseling.

- SEP incluces listening effort, pronunciation, word choice

- grade: pass or fail

- must pass all 3 components (ICE, CIS, SEP)

- ICE = Integrated Clinical Encounter

- CIS = Communication/Interpersonal Skills

- SEP = Spoken English Proficiency

- pass rate: US students: 96%

HOW YOU WILL BE EVALUATED DURING THE PHYSICAL EXAM

- graded for doing a specific maneuver and doing it right

- will not get credit for doing extra maneuver or examining extra system

- you will not get points if you fail to do a particular required maneuver

Preparation for the Step 2 CS (Clinical Skills)

Prep advice

- Recognize that practice is the best way to improve your performance, efficiency, and sense of timing

- UW + FA = enough

- go through 50+ cases, you’ll be fine

- do timed cases too

- The most difficult by far was waiting for the results. After you exit the center all you remember- mistakes, mistakes and more mistakes. I was sure I would not make it

- Do not panic

- learn the objectives for each patient encounter

- develop your time mangaement

- take focused histories and physical exams

- the exam simulates clinical encounters found in clinics and emergency departments

WHAT TO EXPECT

- Examination with 11-12 SPs

- videotapes are not used for scoring

Educational Objectives:

At the end of this lecture, the audience will

- Have a greater understanding of the risks of public disclosure of mental illness.

§ Have a better understanding of the complexity of responses of colleagues.

§ Have a better understanding of benefits of public disclosure.

Audience: Grand Rounds presentations are for educational purposes and intended for a professional audience.

This UCSF CME educational activity was planned and developed to uphold academic standards to ensure balance, independence, objectivity, and scientific rigor; adhere to requirements to protect health information under the Health Insurance Portability and Accountability Act of 1996 (HIPAA); and include a mechanism to inform learners when unapproved or unlabeled uses of therapeutic products or agents are discussed or referenced.

Accreditation Statement:

1) The University of California, San Francisco School of Medicine (UCSF) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

UCSF designates this educational activity for a maximum of 1 AMA Physician’s Recognition Award (PRA) Category 1 Credit(s)TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

2) Nursing: For the purpose of recertification, the American Nurses Credentialing Center accepts AMA PRA Category 1 Credit TM issued by organizations accredited by the ACCME.
