Observed Structured Clinical Exams (OSCE)

During the OSCE, students will be expected to be able to demonstrate all of the PE manuevers that are on the adult PE checklist.  You will not be given the checklists ahead of time.  Rather students are expected to know what portions of the PE to perform given the chief complaint or patient situation.

Remember that the appropriate PE may not fall neatly just into one system.  Someone in
whom you have concerns for congestive heart failure would require as part of
their “cardiac” exam, the cardiac exam, lung exam to check for pulmonary edema,
abdominal exam to check for liver enlargement, and an extremity exam to check for
edema.  It all depends on your differential and what information you need to collect to
figure out the diagnosis.

Here is some information to help address questions that generally come up regarding
the purpose and details of the OSCE. 

What is the purpose of the OSCE?

The OSCE is the final exam on the subject of clinical skills.  Its purpose is to assure that all
students are clerkship ready in basic interviewing and physical exam skills.  It is an opportunity for medical schools to identify and help those who may need additional preparation prior to starting the clerkships.  It is also a learning experience to help you consolidate your skills.  All students must pass the OSCE in order to start their clerkships.

What happens at the OSCE?

Each student will be assigned to a half day for the OSCE. (more or less).  During that time, students will rotate through +/- 6 different examination stations.  All stations are about 35
minutes in length with 20 minutes for the patient interaction, 8 minutes for a written exercise, and 7 minutes for feedback. There will be a combination of standardized and real patients, and some of them will have physical exam findings.

The stations represent patients with chief complaints and fall into 3 types:
1. Communication Skills stations (similar to your FPC small group exercises)
2. Physical Exam Skills stations
3. Focused History and Physical Exam stations
The written exercises vary from station to station, are designed to assess certain areas of knowledge as well as your skills in critical thinking/clinical reasoning and written communications.

A case description with specific instructions for each station will be posted on the outside of the individual stations.  You will not know to what stations you have been assigned
until you check in and you will not know the patient’s chief complaint until you arrive at each station.  You will therefore want to arrive prepared to demonstrate your full range of clinical skills.  The tasks that you will be asked to perform, you have learned in the first two years of medical school. At UCSF, for example, students learn this through FPC
lectures, FPC small groups, PE groups, Integrated Exercises, preceptorships, and other blocks such as BMB and Life Cycle.

How will I be evaluated?

As the final exam for the course, students will be evaluated based on their competency in the five foundations of FPC: professionalism, the patient-doctor relationship, communication skills, physical exam skills, and clinical reasoning.

At each station, a faculty member or standardized patient will be present to provide you feedback on your performance.  Evaluations will be based on your ability to conduct a patient interview/discussion and perform a basic physical exam.  You will be asked to describe, but not necessarily identify, all physical exam findings.  All evaluators will
be using checklists similar to those you experienced last year during the mini-OSCE.  A sample OSCE checklist is sometimes posted for your review.  The actual checklists for each station are secure and will not be released prior to the OSCE.  A numerical score will be generated for each student.

Performance on the written exercises will be assessed on a three point scale:  above expected competency, at expected competency, and below expected competency.  These points will be added to your checklist scores to generate your final OSCE performance score.  No student will pass or fail solely based on the points from the written exercises.

How should I prepare?

The best preparation for the OSCE is to review and practice your interviewing and physical exam skills using the Adult PE Review checklist and the attached sample OSCE checklist from 2008.  You are encouraged to practice with friends, during the Adult PE Review and
Self-Assessment exercise, and at the Life Cycle Integrated Exercise session.  You may also wish to review the Bates videos.

Because you will be seeing patients, you will need to be dressed professionally, similar to your preceptorship dress with your white coat and name tag.  You will also need to bring your stethoscope, reflex hammer, tuning fork, and visual acuity card (all other equipment will be provided in the examination rooms).  Please make sure that you arrive on
time.  Late arrival reflects poorly on your professionalism and lost station time may not be made up.

When do I get my OSCE score and what happens if I don’t do well?

OSCE scores will be posted approximately 2 weeks after the exam.  In addition, students who have been identified by faculty as having areas of concern in their performance will be notified separately by email. 

For students who do not perform at expected competency levels, we will work with you to arrange for special preceptorship and teaching in your areas of need before the clerkships start.  We scheduled the OSCE with enough time before the clerkships begin to assure that anyone who needs further preparation will be able to complete it well in advance of the
clerkships.

Remember, the OSCE may also be a wonderful learning experience.  The evaluators will usually be giving you feedback as you go through each station.  Students regularly report AFTER the OSCE that they both enjoyed and learned a great deal from it.

Advice for the Life Cycle block at UCSF medical school

Advice for the Life Cycle block at UCSF medical school

This block is considered  the “Life Cycle/Epilogue/FPC” and is the last full-length block of the Essential Core curriculum. As usual, the medical students are taught by many excellent faculty instructors. Furthermore, there is a terrific group of MSIV students who will be
teaching in Life Cycle and also serving as mentors to assist students with planning for USMLE Step I and their 3rd year.

One important early point to note is that there is a required textbook for Life Cycle that you will want to pick up. It is Langman’s Medical Embryology, 10th edition, by TW Sadler, Lippincott Williams & Wilkins, 2006. (There might be a newer edition now.) The book comes with a very useful CD of animations called Simbryo. You will want to pick it up early as embryology will be one of the first topics in the course.

The directors of Life Cycle are Susan Masters and Linda Hopkins.

There is one copy of Langman on two-hour reserve in the library (under IDS 107). Version 2.0 of Simbryo is also available on-line (from campus or with vpn). Go to the “Useful Web Sites and Resources” section of the iROCKET Materials/Assessment page and click on the link. That section has links to other helpful embryology websites that the faculty and
students have found over the years.

Some students like to think of this book as a hybrid of the regular ABSITE Review book and the Senior ABSITE review book. It is also significantly more in depth in each of the categories.

This is not a book you can cram at the end of January. It will take you significantly longer to read through it.

Senior surgery residents have found this book extremely useful.

The book is 450 pages, a length that some students find this too long.  One student writes: “I re-opened my new comprehensive ABSITE review today, and it just really seems long. I tried to envision myself getting through 450 pages of dense material in the last week of January, and I just don’t see it happening.”

As far as the little mistakes go, I think that Fiser has less than most study resources. Also, most of the mistakes are really just outdated recommendations. Since expert opinions vary, and the ABSITE tends to lag a few years behind the literature, I’m not sure if such corrections would be much help.

Significance of your ABSITE score

The ABSITE score is used by program directors to determine your progression in residency.

Doing poorly on the exam can have negative effects. One student writes: “theres been at least one example of a resident in my program who scored decently on the junior exam and below 20th percentile on the senior test. and disciplinary action was taken against them.”

The Senior ABSITE is significantly different from the Junior ABSITE and students struggles to reconcile those differences.

One student writes: “All those above me have that skimming through Fiser and doing the usual minimal things that i do for the absite and get by will not work.”

How to study for the Junior ABSITE

Surgery residents are expected to take the ABSITE exam each year of residency. First and second year general surgery residents take the Junior ABSITE. This exam is mostly basic science, with some clinical questions.

There are some who are extremely talented, and can take 1 week to study for the exam.

Does your score matter? Absolutely, and how well you do relative to your peers is what is important. If you score below a certain percentile set by your residency program, you might be placed on probation, and/or fired from your program.

Is my Absite score used in fellowship applications? Well, some residents seem to think that it does matter. Granted, who you know and where you trained matters significantly more than your ABSITE score.

Do preliminary residents have to take the exam? This depends on the program in which you are in. Of course, if you’re a prelim for general surgery, you want to do well. If you’re a prelim for another surgical specialty, your score probably won’t matter, and your goal might be to beat the categoricals (for fun).

“Reading and preparing for your cases and conferences is an excellent way to retain information than just trying to read a text cover to cover. If you can then supplement this with reading from the Fiser absite review, you will do well on the absite. Good luck.”

 ”Reading Schwart/Sabistan are not productive ways of studying for the ABSITE in my opinion. There’s just too much filler in those kind of textbooks.”

This question book contains over 500 practice questions for the ABSITE Exam. It also contains detailed explanations for each answer. It covers all of the major topic areas.

The book is 240 pages and is rated an average 4/5 stars on student reviews.  The questions in this book are simple and basic. It is not as difficult as the ABSITE, but is a great way to reinforce important concepts. 

In summary, this book provides a basic review for topics the ABSITE, but is too simple to rely solely on it for ABSITE review.

Below are some general statements about the topics covered in the ABSITE.

“The ABSITE in-training exam you take yearly during residency contains a fair # of basic-science related to those on the USMLE exams”

 
- The ABSITE covers gastrointestinal physiology and pathology questions. It also covers topics in endocrinology and respiratory system (ex. respiratory quotient has been quoted as being tested each year. ) 

- Other questions that students have mentioned are: “Where is the SA node?” ”How do you revert the protamine induced hypotension?” For that question, one student stated that  Calcium was the closest answer. Another question that students have stated is: “What is the most common side effect of protamine?”

- There are also anesthesia type questions on the exam. One student quotes: “… and then the anesthesia like questions made me want to rather pass ureteric stones. I mean how many malignant hyperthermias have we seen. I havent seen even one.” On this topic, one faculty member wrote:  “Continue to expect those “anesthesia like” questions – you will have them every year.”
- … some of the more esoteric topics (ortho, gyne, etc) that show up on the ABSITE (my chief year ABSITE had like 15 gyn questions, and lo and behold, gyn also showed up on my oral boards!).
- Instead, read the common stuff and know well (Hernias, Choles, Appys, Endocrine). Don’t forget to read and review the Transplant and Oncology as these subjects have loads of Basic Science material to test.

In the senior-level exam, 20% of the exam focuses on basic science and 80% on clinical management. Some facts
- ABSITE stands for The American Board of Surgery In-Training Examination
- It is a multiple-choice examination
- It is designed to measure the progress attained by residents in their knowledge of basic science and the management of clinical problems related to general surgery.
- You can only get your results from your program director

What residents in forums say about the ABSITE
- The ABSITE in-training exam you take yearly during residency contains a fair # of basic-science related to those on the USMLE exams
- It can be heavy on USMLE Step 1 stuff (junior)
- It is more specific toward clinical practice of surgery (senior)
- The exam is somewhat repetitive each year, and that’s where many of these ABSITE guides get their info. There are more than a few questions that repeat every year
- The questions on ABSITE are repetitive.
- PGY 1 & PGY2 Exam version is skewed more toward basic science.
- PGY 3, PGY4 and PGY5 Exam version is skewed more toward clinical science.
