Top 10 things to know about APPENDICITIS for the ABSITE exam

1. Appendicitis symptoms include dull, vague periumbilical pain which then migrates to right lower quadrant (RLQ) and becomes sharp and localized

2. It is associated symptoms include nausea, vomiting, anorexia, and possible fever.

3. Imaging workup is an abdominal CT scan with oral (or rectal) contrast

4. About 10% of the population will develop acute appendicitis during their lifetime.

5.  It is the most common cause of acute abdomen requiring surgery.

6.  Infants and elderly have a much higher morbidity and mortality .

7. Overall mortality is 0.1% for nonperforated cases and 5% for perforated cases

8. In adults, cause is from to a fecolith. In kids, lymphoid hyperplasia accounts for 60% of cases.

9. Physical findings: tenderness at McBurney’s point, with cough (Dunphy’s sign). Rovsing’s sign=push on left, hurts on right. Obturator sign=pain after flex hip, internal rotate hip=pelvic appendicitis. Psoas sign=extend hip=retrocecal appendicitis.

10. In pregnancy, maternal mortality is 1-2%, but fetal demise is 2-9%, up to 35% with perforation.
